
DOCUMENTS TO BE ATTACHED WITH THE MEMBERSHIP FORM
To obtain membership of WUS Health Centre, a prescribed application has to be filled up by University employees/students along with payment of the prescribed subscription fee. The application form can be downloaded from the website of W.U.S. Health Centre http://healthcentre.du.ac.in/Forms.html
- University Employee :
(a) In-service employees :
The membership of above Health Centres may be obtained by the University/College Teacher/Employee on filling up the prescribed form duly verified by the concerned Establishment Branch/College of the concerned Institution. The subscription for membership, along with the application form, for initial one month has to be deposited in the Health Centre office (Room No. 34) between 10:00 am to 12:30 pm through online mode. The requisite monthly HCC for subsequent months by the concerned institution according to the prescribed rates of the University will be remitted each month to the Registrar’s Account. Monthly Schedules exhibiting HCC along with token numbers from Finance – I/Finance – II/ College are to be forwarded to the WUS Health Centre immediately.
The Employees are required to submit properly verified Application Form from their respective Department/College along with following requisite documents:-
- Copy of Aadhar Card/Birth Certificate/10th School Certificate for the dependent beneficiaries.
- Disability certificate of child issued by Competent Medical Authority, if applicable.
- Certificate from the Employer of a working spouse (whether Government or Private Organization) that s/he is not availing any medical facilities from her/his office.
- Affidavit in respect of dependent member(s) on stamp paper (non-judicial) worth Rs. 10/- (Rupees ten only) certifying that s/he is unmarried, unemployed and totally dependent upon the employee.
- Copy of the payment receipt of prescribed fee (payment is to be made on University website http://fee.du.ac.in/index.php/site/login).
(b) Retired employees :
S/he is required to submit an application form duly verified from the concerned Establishment Branch of the concerned institution along with the following documents:
- Copy of Pension Pay Order (PPO)/Retirement Orders and No Dues Certificate issued by WUS Health Centre.
- Copy of Aadhar Card/Birth Certificate/10th School Certificate of the dependent beneficiaries.
- Disability certificate of child issued by Competent Medical Authority, if applicable.
- Certificate from the Employer of a working spouse (whether Government or Private Organization) that s/he is not availing any medical facilities from her/his office.
- Affidavit in respect of dependent member(s) on stamp paper (non-judicial) worth Rs. 10/- (Rupees ten only) certifying that s/he is unmarried, unemployed and totally dependent upon the employee.
- Copy of the payment receipt of prescribed fee (payment is to be made on University website http://fee.du.ac.in/index.php/site/login).
The Level of Pay and Pay Band at the time of retirement must be mentioned clearly on the Application Form.
In case of the Retired Employees 10 years subscription is taken as Life Membership fees. The amount has to be paid through online mode and receipt has to be deposited in the Office of the W.U.S. Health Centre.
- Bonafide Students registered in full time Regular Courses :
Students are required to submit the prescribed duly filled Application Form and authenticated by the Head of the Department/Principal of College along with the following documents:

Students are further required to deposit the prescribed fee through online mode as indicated in the Application Form and receipt has to be deposited in the Office of the W.U.S. Health Centre.
- Students registered in diploma/part time/correspondence courses are not eligible for WUS Health Centre Membership.
